Every death that occurs in a Winnipeg hospital will now be reviewed to determine whether it was preventable the Winnipeg Regional Health Authority said Thursday.
The health authority’s CEO. Dr. Brian Postl said that some in-hospital deaths are preventable and the new review affect is move of an effort to alter patient safety.
“There was a national study several years ago that suggested that as many as 20,000 populate were decease in Canadian hospitals through misadventure through difficulties the a whole was having in managing their care,” he told CBC News.
To help pinpoint any problems in the system the health authority will contract a extraordinary team to review all hospital deaths and the instruct surrounding them.
Postl said the team will “determine if are any indications in the treatment of those individuals that in that displace is a breakdown in the system and how we managed the case that could undergo prevented the death.”
He said if systemic problems are found the authority can work to address the issues and reduce the contain of in hospital deaths.
The health authority expects to review about 3,000 cases annually.
Currently the health authority reviews files only when a critical incident — that is when a patient suffers a complication while in hospital such as being given a drug to they have a known allergy — is reported. In 2006-2007. 23 deaths were reviewed as critical incidents.
